Our Hospital to Home service is tailored to people returning home after a hospital stay. It is a temporary package provided by Right at Home Alton and Bordon which is aimed at helping clients settle back at home safely and comfortably.
Who is it for?
This service is for adults of any age needing care to help their rehabilitation after a hospital stay. We regularly work with clients leaving the Royal Hampshire County Hospital and Basingstoke and North Hampshire Hospital among others. We support clients living in Bordon, Alton, Hindhead, Churt, Bentley and nearby areas.
Often our clients are elderly people preparing to leave hospital after a stroke, a fall or surgery. However, we can also support clients of other ages needing care following, for example, cancer care or an operation or injury.
Sometimes, clients simply want the reassurance and companionship of having someone to help them as they adjust or regain their confidence.
Each Hospital to Home package is personalised to the individual client and their particular needs relating to the reason for their hospital stay, their level of ability, confidence and the type of support they need.
How Hospital to Home works
At Right at Home Alton and Bordon, our aim is for a seamless transition from hospital care to living at home:
- Our care manager will carry out two assessments, working with the client: the first in hospital and the second at the client’s home.
- Prior to the client’s discharge from hospital, our care manager will develop a tailored care package to be discussed and agreed with the client and their loved ones, if appropriate.
- From discharge, Right at Home Alton and Bordon will provide care in line with the care package, adapting as the client’s needs change.
The minimum time for our care is one hour but flexibility is at the core of Hospital to Home and each client’s need will be different. Our services extend to live-in and overnight care.
Additional care services include:
- Understanding medical equipment, such as catheter care
- Linking with district nurses and occupational therapists
- Providing family members with support such as moving and handling, continence information and dementia support.
Benefits of Hospital to Home
Hospital to Home can help fill in any gaps left if family members or friends are not able to provide care themselves. For clients whose families do not live nearby, this provides much needed practical help as well as welcome reassurance.
Having a care package in place at home can prevent prolonged stays in hospital and the support of a professional caregiver can ensure that clients take the correct medication or follow the aftercare advised by the hospital.

Hospital to Home is a rehabilitation package designed to be temporary. The aim is to help clients get back to normal, living independently and as actively as possible.
