Working with Virtual Wards - A Triumph of Home-Based Rehabilitation

Client had recently moved to the Nottingham area and experienced a fall that required urgent medical attention. Following assessment by the Urgent Care Team, it was determined that home-based rehabilitation would be the most beneficial approach to recovery, allowing her to remain in the comfort of her home.

In collaboration with the Virtual Ward, our team conducted a comprehensive assessment to identify the Client's specific needs. This initial evaluation revealed the necessity for specialised equipment, such as a rotunda and commode, to facilitate safe transfers. Our team swiftly deployed two dedicated care professionals to provide the necessary home support.

Under the expert guidance of our team and the Virtual Ward, the Client demonstrated remarkable progress. With consistent support and encouragement, she quickly gained confidence in her abilities and began to perform more tasks independently. Her mobility improved significantly, and she was soon able to stand without the assistance of a rotunda.

A major milestone was reached when the client successfully transitioned to using a walking frame for transfers. This marked a significant step forward in her recovery journey, and she was subsequently discharged from the Virtual Ward.

Continued Success:

With ongoing home support, the client's mobility continued to improve. Through perseverance and practice, she was eventually able to walk between her bedroom and the bathroom without the need for a commode.

Outcome:

Thanks to the dedicated efforts of our team and the Virtual Ward, the Client has successfully rehabilitated to her pre-fall functional level. She now requires minimal support and is able to enjoy a fulfilling life within her family home.
