Client PS approached Right at Home Tyneside (RaHT) in early 2017 for care and support, following inconsistent call times and a lack of continuity of care from her then provider and, indeed, her previous provider. PS has MS and an auto-immune condition, which affects her skin integrity and leads to severe outbreaks of extremely painful, fluid filled blisters. The blisters mean that when PS needs to be assisted in dressing or undressing and being moved, there is a significant risk of the blisters being opened, which compromises her skin integrity further and heightens the risk of infection. PS also has severe lymphoedema. Mrs Smith also has a suprapubic catheter and bowel management with district nurse intervention.

Following a spell in hospital, RaHT's managers worked with the hospital moving and handling team and the community occupational therapist to agree an appropriate moving and handling plan that minimised risk to PS, given the conditions outlined above. The Training Manager and Care Manager both attended the hospital to agree the most appropriate moving and handling techniques and to ensure safe practice.

Subsequently, we amended the moving and handling plan to reduce the care team from 3 to 2 members following rehabilitation, which also allowed better utilisation of resources. We have also been able to assist in getting an additional ceiling track hoist fitted to facilitate moving PS between her bathroom and living / sleeping area. PS's call times were coordinated with the district nursing team for bowel management so that the care team would work alongside a district nurse for the moving and handling to increase capacity for the nursing team (previously 3 district nurses were in attendance).

Benefits to PS

The Client's skin integrity has improved significantly, the moving and handling techniques employed, alongside steroid treatment, contributing markedly towards this. PS is also able to wear clothes of her choice, such as trousers rather than skirts, which also assist in ensuring her dignity is maintained when sitting or being transferred. PS is receiving non-problematic catheter and bowel care.


Right at Home Tyneside first supported SK in late 2014, making twice weekly visits to assist her husband with taking SK out, helping her around the house and providing her with companionship. SK was in the early stages of dementia when the support was first commissioned but rapidly progressed to requiring 24-hour support by May 2016. SK's husband wished to oversee his wife's care and decided to employ PA's to facilitate this and our involvement temporarily ceased.
