What is hospital-to-home care?
Hospital-to-home care provides the vital support needed to make a smooth transition from hospital back to everyday life, with recovery taking place in the comfort of home, where you're most at ease.
This type of care is especially helpful after a hospital admission, surgical procedure, fall, or a period of illness. At Right at Home, we work closely with hospital discharge teams, families and Clients to ensure everything is in place before you return home. We then provide ongoing, responsive support, helping you manage medication, prevent complications, and regain independence safely.
Post-operative care at home also includes monitoring changes in wellbeing, ensuring you’re following recovery guidance. It also involves being there to lend a hand (or a listening ear) as you heal. Our approach is practical, proactive and always personal.
Is post-operative care only for older people?
Not at all. While many of our Clients are older adults, we provide post-operative and recovery care for adults of all ages who need short-term support after surgery or illness. Whether you’re recovering from a hip replacement, planned procedure or unplanned hospital stay, we’re here to help.
Can you help with arranging hospital discharge?
Yes. We can liaise with discharge coordinators, family members and health professionals to ensure everything is in place before you return home. This may include transport, mobility aids, meals, and medication, as well as having a Carer ready to support you when you arrive home.
What types of surgery or conditions do you support?
We support recovery following a wide range of medical conditions and surgeries, including orthopaedic operations (e.g. hip or knee replacements), cardiac procedures, cancer treatment, and recovery from falls or general illness. We also offer care for Clients with complex needs, including catheter or stoma care, where trained.

How long does hospital-to-home care last?
It depends on your recovery. Some Clients need just a few visits after returning home, while others may benefit from several weeks of support. We offer completely flexible care options — from short-term visiting care to comprehensive live-in care — and regularly review your needs as your health improves.
Will I have the same Carer each time?
Wherever possible, yes. Continuity of care is very important to us. We do our best to ensure you or your loved one is supported by a small, consistent team of familiar Carers who understand your preferences and recovery goals.
What happens if my condition changes?
We regularly review all Care & Support Plans to make sure they remain suitable. If your recovery progresses or your needs change, we’ll adjust the plan accordingly. Our aim is to provide the right level of support at every stage of your recovery.
